Scholarship Review Form
Below is a list of selection criteria and process as well as the provision of funding
Selection Criteria
Check all criteria that you feel appropriate:
_____1. Should be enrolled as a high school senior or 1st or 2nd year college student with a minimum GPA of 2.5.
_____2. Should be interested in Urban Inspirational programs of study leading to a career in Technology, Arts, Business, Entertainment, Media, or Music.
_____3. Should submit 3 recommendations from academic or extra-curricular coaches or mentors.
_____4. Should submit a copy of individual gifts/talents of performance in the areas of arts, buisness, entertainment, media, or music.
Provision of the Funding
Funding will be on an annual basis at a minimum $1,000 per awarded participant.
Selection Process
UGAI Community Improvement Corporation’s Scholarship Committee will make scholarship elections based upon the established criteria.

Applications must be postmarked by September 30th
Applicants must be available for an interview by telephone or in person.
Scholarships up to $2,000 will be awarded.
Awards will be disbursed in the recipient’s name to his or her educational institution.
